In addition to the base package, the following is usually required:
- a police report or certificate describing the circumstances, with the report's registration number;
- an affidavit (a sworn statement) in which the applicant describes the situation: when, where, and under what circumstances the passport was lost;
- where necessary, additional explanations if the circumstances of the loss require clarification.
It is important that the affidavit and the police report match on dates and details. Discrepancies often lead to requests for clarification and extend the overall timeline.
